PERMISSION TO SKATE REQUESTS - CENTRAL ONTARIO WOLVES AAA

The Ontario Minor Hockey Association (OMHA) requires that all players attending tryouts provide their AAA Zone with a Permission to Skate (PTS) form.  The PTS form must be obtained from their home centre prior to stepping on the ice.



In order to receive a PTS form from the North Durham Minor Hockey Association (NDMHA), players must complete the online request form. The request must include player name, birth date, address, team played for in current year and team trying out for. The Player MUST be first registered with NDMHA for the 2018-2019 season through the Hockey Canada Registry to receive the requested documentation (please select the pay by "Other" option to avoid the $40 administration fee).

NDMHA registration opens April 2, 2018.


Please note that a PTS form is not a player release nor is it an OHF AAA waiver form.

The North Durham Minor Hockey Association can only provide a PTS form for the Central Ontario Wolves for players eligible to play with the Central Ontario Wolves. We cannot provide a PTS form for any other AAA Zone. If a player is released from the Central Ontario Wolves they can request an OHF AAA Waiver form from the Central Ontario Wolves to allow them to tryout with an adjoining AAA zone, otherwise they return to North Durham Minor Hockey Association to play.

PTS Forms will be emailed to you by April 14, 2018. 

Note: 2002 and 2001 players PTS forms for Midget AAA will become available in August 2018.
