Midget Houseleague Medical Info and Concussion Forms
Attached is the Medical Information Form and the Concussion Awareness Resource and Form. These forms are MANDATORY FOR EVERY PLAYER. You cannot be Rostered without submitting these forms. All players must sign both forms and players under the age of 18 must also have a parent's signature on each. If you have not submitted the forms, please bring them to your first team skate.
