NOTE: Division is Combined Port Perry/Uxbridge. The evaluation times below are for ALL PLAYERS.
Once Evaluations are complete, you will be contacted with your team information and first Ice Time.
Team skates will begin on September 24.
First Game times will be posted ASAP.

*IMPORTANT INFORMATION*
New this year, a direction from OMHA came to us with information regarding Concussion Awareness. On September 16, I emailed 2 documents regarding this to all players. Please review the document titled "Concussion Awareness Midget" with your player, and please have your player and a parent/guardian (if under 18) PRINT AND SIGN the Acknowledgement Form. I have also emailed the Medical Information Form. PLEASE BRING THE SIGNED FORMS WITH YOU TO THE FIRST EVALUATION.
You will NOT be allowed to play a game without these forms submitted.
PLEASE ALSO NOTE: FOR THE SAFETY OF ALL PLAYERS, CELL PHONES ARE BEST LEFT AT HOME, AND NOT BROUGHT INTO DRESSING ROOMS.
